Chronic Disease Self-Management Program Innovation Readiness Assessment
 
Thank you for your interest in the Chronic Disease Self-Management Program (CDSMP). CDSMP is an evidence-based program that is offered through a workshop that meets for two and a half hours, once a week, for six weeks. People with different chronic health problems attend the program together in community settings. Workshops are facilitated by two trained leaders, one or both of whom have a chronic health condition.

The Self-Management Workshop will not conflict with existing programs or treatment. It is designed to enhance regular treatment and disease-specific education. In addition, many people have more than one chronic condition. The program is especially helpful for these people, as it gives them the skills to coordinate all they need to manage their health, as well as to help them keep active in their lives. We strongly suggest you familiarize yourself with CDSMP before beginning by visiting the Stanford University website at http://patienteducation.stanford.edu/

Over the next 15-20 minutes, you will be responding to questions aimed at helping you determine your organization’s readiness to implement CDSMP. If you would like to confer with others before beginning, click View File to download a blank copy of the survey. Please choose one person to serve as a point of contact. An email with your results report will be sent upon completion of the online survey. This report will provide you potential actions you may choose to take to enhance your organization’s readiness. You will also have the option to share your report.
