The survey is a baseline of your Lean and Six Sigma knowledge and will be used as a measurement took in preparation for your training and to capture specific needs for your development. There are no foreseeable risks associated with this survey, however this survey is mandatory for the completion of your preporation for training. The survey will be strictly confidential and is used specifically for your development. For information submitted, and projects completed you will be asked to bring completed reports for your Black Belt or Master Black Belt interview, certifications, and/or copies of other documentation as discussed in your survey. The assessment defines the key behaviors that are exhibited by all employees in a Lean Six Sigma Company. It provides a process to help each business unit and function measure how well they are progressing toward practicing Six Sigma behaviors. By analyzing the gaps revealed in the assessment, each business unit can develop a focused improvement plan.



The assessment process calls also for re-measuring and analyzing our behavioral performance on an ongoing basis to ensure control and continuous improvement.



The assessment consists of (22) behaviors aligned with Manitowoc Strategic Imperatives. Current behavior is assessed on a 1(lowest) to 5 (highest) scale. A description of the behavior at each of the 5 levels is provided to ensure consistent evaluation.

1. As a leadership team, we regularly review a variety of customer inputs to ensure we clearly understand our customers's requirements.
2. We use employee, customer and process performance data to prioritize improvement opportunities and projects.

11. We assign process owners to key processes, giving them clear accountability for monitoring process performance, achieving results and continuously improving.

16. We encourage sharing of effective practices and Lean Six Sigma solutions with other units by providing full documentation and the use of our resources.
