My name is Kelly Carroll,a regional manager of Kootenay Savings Credit Union. I am completing a strategic business plan project which forms part of the requirement for a Masters in Business Administration at Royal Roads University. A component of this research will include this survey, which is foreseen to take 5 minutes to complete. The questions will refer to the level of Kootenay Savings brand awareness in the East Kootenays. In addition to submitting a final report to Royal Roads University in partial fulfillment for an MBA, I will also be sharing my research findings with the Credit Union through the development of a management report.

The information you provide will be summarized, in anonymous format, in the body of the final report. At no time will any specific comments be attributed to any individual unless your specific agreement has been obtained beforehand. All documentation will be kept strictly confidential. In the event that you reside in the United States, you are advised that its governments, courts, or law enforcement and regulatory agencies may be able to obtain disclosure of the data through the laws of the United States.

You are not compelled to participate in this research project. If you do choose to participate, you are free to withdraw at any time without prejudice. Similarly, if you choose not to participate in this research project, this information will also be maintained in confidence.
