Hello Business Office Team!

You are invited to participate in our survey surrounding Employee Engagement. In this survey, you will be asked to respond to questions about Employee Engagement, as well as to choose from your teammates for recognition in four categories. It will take approximately ten minutes to complete the questionnaire.

The first two questions will allow you score the response on a scale from 0 to 5.  Please think well and evaluate your opinion as they relate to the question in your overall experience in the subject matter indicated.  The latter four questions will give you the opportunity to choose a colleague in a specific category, which will be further elaborated in the question.  The categories are: "Most Collaborative", "Adaptability", "Student Service", and "Above & Beyond."  Note that it is possible that a single person could win multiple categories; we are allowing you to choose whomever you feel is best in each on their merit in the specific category.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
