Greetings,
You are invited to participate in an important anonymous survey on Opioid pain medication education. In this survey, which will be sent to all Western Michigan Athletic Directors, there will be questions in regards to your knowledge and education on Opioid use/abuse in high school athletes.
I have chosen Opioid use/abuse in high school student athletes as my research project to complete my Master’s degree in Health Professions Education at Michigan State University.
The results of this survey, while anonymous, will be utilized to offer further education and awareness.  This survey will take between 3 to5 minutes to complete. Your participation in this study is completely voluntary and again you will remain anonymous. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.
