Along with an in-person discussion on September 16th, this short board self-assessment tool will help us determine our governance needs and strategic planning readiness for the organization.

- This survey tool is intended only for current Board members of Community Care Northumberland
- It will take approximately 5-7 minutes
- Please complete before 5pm on Friday, September 2nd, 2016
- There are multiple choice-type questions, and a space available for written feedback at the end
- This survey tool is anonymous and is being conducted by Laridae Communications Inc. who will provide the results to us as grouped data. The results will be used to prompt and guide our board discussion.
