Enjoy Church East is conducting a survey with people who attend the church to learn more about every individual and their abilities.  We also aim to discover ways we can support the church and individuals in reaching out to the communities that we belong to.  We are interested in learning about who you are, what you are interested in, what skills, expericnce or knowldge you have and what communities you are in contact with in your life.

You are invited to participate in this survey that will take between 10-20 minutes complete.  Please feel free to approach us in the months of ... to ... if you would like to add or change the answers you provide today.  Your participation in this study is completely voluntary and you may request to withdraw at any point throughout the survey.  

We wish to advise you that this information is confidential and will be kept in this program that is password secured and is not linked with your personal details aside from your name you provide today.  Important information may be shared with the database program that Enjoy Church owns upon your consent.  Church leaders and those involved in this team are able to access the information you provide today.  If you have questions at any time about the survey or the procedures, you may wish to speak to Rachel and Hayden Jones.
