This survey has been commissioned by the Board of Epsom Sports Club Ltd, and is very important for the future of Epsom Sports Club and all the sections. As a member, or parent of a junior member, your response is highly valued and much appreciated. The Board welcomes the participation of all players, parents of juniors, non-playing and social members.
This survey is being carried out by an independent researcher, and is confidential. The Board will only be shown aggregated data from which it will be impossible to identify individual responses. If, however, you wish to disclose your identity you may, as a further consultation may be necessary.
We recommend that members under 13 complete the survey in conjunction with a parent, but that the parent also completes a separate survey too.
For households with multiple Section or Club members, once a survey has been submitted by one individual another fresh survey can be opened and completed by different respondent.
