You are invited to participate in our survey. The Farm-to-Fork Program is a program being jointly developed by the California Department of Education, California Department of Public Health and the California Department of Food and Agriculture. The program was established per the Governor's Executive Order S-04-10, under the auspices of the Strategic Growth Council (SCG) as a multi-agency collaborative effort to improve state policy and decision making in working towards health and sustainability goals by incorporating health considerations into non-health policy areas. The Farm-to-Fork Program was one of the eleven approved priority recommendations which will encourage and expand the availability of affordable and locally grown produce through Farm-to-Fork policies and programs.
It will take approximately 10 minutes to complete the questionnaire.
