You have recently been involved or are presently involved in a Teva clinical trial and as such, we would like to get your feedback on the CRO Team.

Teva Pharmaceuticals is constantly striving to improve the performance of its CROs and would appreciate if you would take approximately 5 minutes of your time to complete the attached questionnaire. The results will be used to understand your satisfaction with the CRO, which represents Teva.

Your survey responses will be coded and will remain strictly confidential. Data from this survey will be reported only in aggregate results.
