Your organization has been chosen to pilot a new non-mandatory tool for individuals with disabilities (and those who support and know the individual) who are interested in competitive employment.
Designed through the collaborative efforts of the Department of Assistive and Rehabilitative Services (DARS), the Department of Aging and Disability Services (DADS), the Department of Family and Protective Services (DFPS) and the Department of State Health Services (DSHS), the tool will assist the team, including the individual, to consider and identify interests, strengths and supports available when employment has been chosen as the service option.
We would like you and all staff providing employment services; or if you are with DFPS are making referrals to employment services in the pilot areas; to complete this survey by January 4th at close of business.
Neither the survey nor the pilot is mandatory, but your valuable input and participation will help the individuals we all serve to achieve their employment goals!
Your participation in this survey is completely voluntary.You can withdraw from the survey at any point if you feel uncomfortable answering the questions.
