Permission Form of Implied Consent Dear Survey Participant: I, Brittney Saunders am conducting a research project entitled “Health Care Providers’ Preference for Medical Models of Care and Their Impact on Care" as part of a class project that will later turn into an independent study at South Dakota State University. The purpose of the study is to explore the difference in medical communication model preferences, the way medical professionals communicate with patients, and how characteristics such as career, age, experience, and gender affect the communication process. You as a medical professional are invited to participate in the study by completing the attached survey. I realize that your time is valuable and have attempted to keep the requested information as brief and concise as possible. It will take you approximately 10 minutes of your time.
