Permission Form of Implied Consent

Dear Survey Participant:

I, Brittney Saunders am conducting a research project entitled “Health Care Providers’ Preference for Medical Models of Care and Their Impact on Care" as part of a class project that will later turn into an independent study at South Dakota State University. The purpose of the study is to explore the difference in medical communication model preferences, the way medical professionals communicate with patients, and how characteristics such as career, age, experience, and gender affect the communication process.

You as a medical professional are invited to participate in the study by completing the attached survey. I realize that your time is valuable and have attempted to keep the requested information as brief and concise as possible. It will take you approximately 10 minutes of your time. Your participation in this project is voluntary. You may withdraw from the study at any time without consequence.

There are no known risks to you for participating in this study. There are no direct benefits or rewards for completing this survey.

Your responses are strictly confidential. When the data and analysis are presented, you will not be linked to the data by your name, title or any other identifying item.
