Informed Consent Statement

Project Statement:Getting People to Leave their Homes during a Hurricane
I invite you to participate in a web-based online survey that is completely anonymous. I am looking to reach all students, faculty, staff or anyone who is regularly living in a hurricane prone environment that have experienced a hurricane and have possibly left or stayed home during a hurricane.
The survey should take less than seven minutes to complete. 

Participation in this research project is voluntary:
This survey is completely voluntary on your part. There are no benefits from the survey once taken. Again, the survey is completely voluntary.

Your information will be kept confidential.
The web-based survey is administered through QuestionPro. QuestionPro does not collect or maintain any information that would identify who has taken the survey. Only the answers to the questions are recorded. There is no possible way for anyone to know what you answered to the answers on this survey. 

Statement of Risk:
There is no foreseeable risk other than the time it will take you to take this survey.
