As a ministry of excellence, we want to continue doing what we do well, and improve in areas where there may be deficiencies. Our goal is to achieve and maintain a “5-Star Ranking” Ministry that delivers superior customer service to our partners.

PURPOSE:
This survey is designed to provide feedback regarding your role and experiences as a Ministry of Helps Worker within the department(s) you are currently working in at SOFCC. Your participation in this survey will enable us to identify what we do well as a ministry organization and identify areas that may need improvement.

ANONYMITY:

This survey is being distributed to all Ministry of Helps Workers. Your responses to this survey will be completely anonymous. No name is needed, but identifying the area of ministry (at the end of this survey) in which you support is important. All completed surveys will be analyzed and results will be reported in general terms with the ministry at large.
