PS3 Defect Discovery

There has been a very large number of reported users with dead bluray drives in their ps3s. The symptoms are always the same; upon insert of a DVD/Bluray/PS3 game/CD the access indicator spins in the upper right-hand corner, but the disc never appears in the XMB menu. We are beginning the process of a class action lawsuit, and need your help. By answering the following questions, you agree to let us list you as a contact for the class action suit that we're preparing for Sony.

This survey assumes that you purchased a Play Station 3, and subsequently experience a failed Bluray drive.

Before we being, can you tell us if you have done any of the following:

Did you open, modify the PS3 prior to the drive's failure?

Did you otherwise void the warranty prior to the drive's failure?

Did you bump, drop, or otherwise abuse your PS3 prior to the drive's failure?

The Majority of reported drive failures are related to a game locking up the system, the power button on the front of the PS3 is unresponsive, so the user turned off the power on the PS3 by flipping the power switch off on the back of the unit. Did this happen to you?

What PS3 model are you having this problem with? (If you've had this problem with multiple SKUs, please list the most recent model you've experienced this problem with.)
