We would like your assistance with a very short survey we are conducting into the effect of different types of incentive on participation in research programmes. The survey has only seven questions and should take less than ten minutes to complete. In return we will enter your details into a draw for a £50 Amazon voucher. We are sending this survey to a sample of fewer than 400 individuals and we need completed responses by 8.00pm on Sunday 23th October.
Your individual response will be kept entirely confidential. We ask for your name and email address to ensure responses are received only from invited participants and for entry into the prize draw - please note only surveys completed by invited participants will be entered into the prize draw.

3) We are looking at three types of incentive that might be offered at different levels and in different combinations. These are:
i) A guaranteed reward for participation in the form of an on-line voucher for a vendor such as Amazon, Marks and Spencer or Tesco.
ii) Entry into a prize draw for a substantially higher value voucher from the same vendors.
iii) A charitable donation to Macmillan Cancer Support, NSPCC or the Disasters Emergency Committee.
The list below shows different combinations of incentives for participation in surveys of different lengths. For each combination of please indicate how likely you would be to participate on a scale of 1-10. Please also assume that the subject of the survey is of moderate interest to you.
