The Southwest/West Central Service Cooperative (SW/WC SC) is conducting a survey of member districts and service providers in order to help us determine if the alternative programs provided by the service cooperative are meeting district needs as they relate to students who require alternative programming options. As someone who works closely with the students served in these programs, you have important insight into how effective the services provided by the alternative programs are. In this survey we are hoping to collect your perceptions about the effectiveness of these alternative programs. The results of the survey will be used as part of a comprehensive study designed to help the service cooperative make program and personnel improvements if needed, so as to better serve the students and our member districts.
Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. If you feel uncomfortable, however, answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions. The survey will take approximately 5-10 minutes to complete.
Your survey responses are completely anonymous and individual responses will be kept strictly confidential. Data from this research will be reported only in the aggregate.
