2012 Timber Lake Community Health Assessment
Individual Questionnaire
The intent of this survey is to gather answers and ideas from Timber Lake Community and surrounding rural area for residents ages 55 and over to look at the overall health of the community so indicators can be acknowledged that make it an age-friendly community. The end goal is that over time individuals; groups of community members; businesses; organizations; and local government can address needs that may be identified to help make the Timber Lake Community and surrounding area a healthier place where residents are physically active, have lower incidents of chronic disease. Where we live affects our health and well-being so it is important as a community, we build on our strengths. A community that works for persons over 55 works for everyone!

The Timber Lake Community Health Assessment is made possible with a Community Assessment Grant from the South Dakota Department of Health and administered by Missouri Breaks Industries Research Inc. with the support and guidance of a Community Health Team comprised Timber Lake Area residents.
