Exit Survey

Hello:
You are invited to participate in a survey on the homemods.info website.

The survey is being undertaken by the Physical Disability Council of NSW (PDCN), the peak body representing people with physical disability across New South Wales. This survey aims to evaluate the usability of the homemods.info website for the Home Modifications Clearinghouse, University of New South Wales. This website aims to provide evidence-based information on Home Modifications in a usable format and has already been audited for accessibility by Vision Australia. The survey by the PDCN aims to further enhance the usability of the site for people with physical disability.

Approximately 15 people will be asked to complete the survey which will take approximately 60 minutes to complete.

The survey has four parts:

Firstly, there are some questions on your background that we need to develop a profile of people who use the website.

Secondly, you are asked to participate in 5 tasks which involve finding information on the website. You will be given instructions on the tasks and you can contact Annette Mitchell by email at [email protected] should you encounter any difficulties.

Thirdly, you will be asked some general questions about your experience on the website, involving any difficulties you encountered or any issues in using the site as well as ways you resolved these difficulties or issues.

Finally, you will be asked to rate your level of satisfaction with using the website.

Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.

Your survey responses will be strictly confidential and no identifying details will be published. The report and any subsequent publications will use a pseudonym. If you have questions at any time about the survey or the procedures, you may contact Annette Mitchell at [email protected].

Your commencement of the survey indicates that you understand:
a) the background and reason for the survey;
b) you can refuse to answer a question or stop the survey at any time;
c) you are giving consent to use your details and information provided no identifying details are published and that a pseudonym will be used.

Thank you very much for your time and support. Please start with the survey now by clicking on the Continue button below.
