This questionnaire is part of a study directed at veterans’ satisfaction with the Department of Veterans Affairs (VA). Recently, there have been several complaints with issues dealing with wait times to get into a Veterans Affairs Hospital. We would like to know if there are many veterans who are having problems with receiving adequate care at their local VA hospital.
You have been carefully chosen as part of a representative sample of the veteran who use the VA hospital in Virginia Beach, Virginia. As someone who uses the facilities at the VA hospital, your participation in this survey is very important so as to figure out how satisfied with the care they receive. The questionnaire takes about 10 minutes to finish. Your identity will not be disclosed in anyway, the entire survey is entirely anonymous.
The Results of the study will be published and the Department of Veterans Affairs will see the outcome. The actual Data, though, will only be reported as a group, and your identity will not be disclosed, nor will you be identified. No data from a specific veteran or specific VA hospital will be released.
