You are invited to participate in our survey [Westlake Parkinsons Support Group Planning for 2014]. We have started planning our agenda for 2014 and would like input from you to give us direction. Please fill in the short survey below.

Thank you for your time. This is my first time using this program. Please forgive any errors that I may have missed. Our support group is operated by volunteers. If you have a good suggestion or idea that requires some time and effort to plan, we may ask you if you would be willing to participate and make necessary arrangements.
