You are invited to participate in our survey "Woodside Swim Team". In this survey, approximately 200 Woodside Swim Team families will be asked to complete a survey with questions regarding many aspects of the Woodside Swim Team program this past summer of 2013. We look forward to your responses so that we can continually strive to improve our program.

Your participation in this study is completely voluntary and you can withdraw from the survey at any point. It is very important for us to learn your opinions.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
