Dear Participant:



My name is Marvin Bethel and I am a graduate student at Central Michigan University. As part of my research, I am examining the correlation between job fulfillment and work performance. I am inviting you to participate in this research study by completing this survey.


It will take approximately fifteen minutes to complete the questionnaire.

There is no compensation for responding nor is there any known risk. In order to ensure that all information remains confidential, please do not include your name. A copy of the project will be provided to Central Michigan University and may be used upon request, in assisting management decisions within the Southwest Civilian Operation Center organization. Participation is strictly voluntary and no official time will be used to complete the survey.


However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.



Your responses to this web-based survey will be anonymous. In addition, the data from this research will be reported only in the aggregate and destroyed by June 15, 2005.
