Dear Management Academy Alumni:




Hello! We are present members of the Spring 2005 Management Academy session, and our team project is called "Management Academy Evaluation." We would like to invite you to participate in our evaluation survey.



In this survey, you will be asked questions about the impact that your participation in the Management Academy had on you and your department, how you incorporated these new skills in your department, and what specific things you learned that you found to be the most (and least) useful. It will take approximately 15 minutes to complete the questionnaire.



Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. It is very important for us to learn your opinions. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point.



Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential.
