Sample Survey

Hello:

You are invited to participate in this survey on Employee Loyalty. The survey forms part of my MBA Dissertation, and should take only 5-8 minutes to complete. All responses are completely confidential and non-attributable, as no personal information is required to participate.

The Dissertation investigates factors which create Employee Loyalty in 21st century businesses. There have been radical changes in the business environment during the last 10-15 years, and Employee Loyalty has perhaps taken on a new lease of life. Has it affected you in your current or previous jobs? Your input will assist greatly in obtaining an up-to-date perspective, and if there are any other factors not mentioned here that you have experienced, I would be delighted to hear from you.

Your participation in this study is, however, completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point.

Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate. Your information will be coded and will remain confidential. If you have questions at any time about the survey or the procedures, you may contact Paul McIntosh by email at the email address specified below.

Thank you very much for your time and support. Please start with the survey now by clicking on the Continue button below.
