EPS Questionnaire

Hello

I am a third year Computer Science student at Lancaster University.

As part of my dissertation I am studying the Electronic Prescription Service, which is part of the NHS National Programme for IT and the new pharmacist contract.

Enclosed in this envelope are two questionnaires, one aimed towards a pharmacist and one towards a dispenser. I would be very grateful if you or staff within the pharmacy could take the time to fill out the questionnaires.

Each questionnaire should take approximately five minutes.
You will find a self-addressed, pre-payed envelope enclosed to return the questionnaires.

ALL QUESTIONS ARE OPTIONAL AND EVERY QUESTIONNAIRE IS CONFIDENTIAL.

If you have questions at any time about the survey or the procedures, you may contact Emily at [email protected]

Thank you for your time to help with my research.
