INFORMED CONSENT


You have been invited to participate in a research study of the Greater Binghamton Region. We hope to learn about employers’ perceptions of supported employment. Supported employees are disabled individuals who may need ongoing support services in order to perform their jobs. You were selected as a possible participant in this study because you are a Human Resources Manager/ Hiring Manager of a Greater Binghamton Business/Company.



If you decide to participate, we will ask you to take a short anonymous internet survey. The survey will take approximately 5 minutes to complete. The purpose of this study is to understand employers’ perceptions of hiring individuals with disabilities and experiences when working with supported employment agencies. There are no foreseeable risks incurred for participating in this study. Completing the survey may provide an opportunity for you to reflect upon hiring practices. It also will provide valuable information to local supported employment agencies.



The information collected from this survey will be kept anonymous and confidential. Your responses will not be linked to your name or your workplace. By completing the survey, you agree to participate in this study. The data collected will be presented to the community and local supported employment agencies. The research data will then be archived at Center City Coordination (C3) Program. The purpose of C3 is to help revitalize neighborhoods by improving the overall quality of life for residents.



Your decision whether or not to participate will not prejudice your future relations with Binghamton University, C3, or the Supported Employment Agencies in the area. If you decide to participate in this short survey, you are free to withdraw your consent and discontinue participation at any time without prejudice.



Before you begin the survey, please ask questions on any aspect of the study that is unclear to you.
