The following survey questions are designed to obtain information about customer service experiences. The following questions ask you to recall (or imagine) a typical experience in a casual dining restaurant*. This is an anonymous survey—your responses will be kept confidential and will be combined with others. This information is used only for research purposes.


It is important that you answer every item—the survey consists of approximately 40 items and should take about 20 minutes. Please respond to each item honestly and accurately. Thank you for both your time and consideration.


*A casual dining restaurant is a restaurant that serves moderately-priced food in a casual atmosphere. They typically provide table service, and may serve beer or wine or have a bar adjacent to the dining room. Applebee’s, Chili's, T.G.I. Friday's, and Olive Garden are typical examples of a casual dining restaurant.

Suppose you have just received your bill at a casual dining restaurant and you are paying. The service was satisfactory, you are dining with a friend or relative, and there is no special occasion. The total bill, excluding tip, is $30. Please enter the tip amount (in dollars and cents) that you would normally pay (ex. $1.23).
