The purpose of this survey is to understand how you feel about eating at local restaurants. Your responses are extremely valuable to furthering our research. It will take approximately 10 minutes to complete this survey. You must be at least 18 years old to participate and your participation is purely voluntary.


The survey poses no risks to your well-being. Your survey responses are strictly confidential and data from this research will be reported only in summary format. Your name or specific answers will not be shared with anyone. If you feel uncomfortable answering any question, you can withdraw from the survey at any point without penalty.
