It will take approximately 10 minutes to complete this survey. You must be at least 18 years old to participate and your participation is purely voluntary.
The survey poses no risks to your well-being. Your survey responses are strictly confidential and data from this research will be reported only in summary format. Your name or specific answers will not be shared with anyone. If you feel uncomfortable answering any question, you can withdraw from the survey at any point without penalty. The following survey questions are designed to obtain information about customer service experiences. The following questions ask you to recall (or imagine) a typical experience in a casual dining restaurant*. This is an anonymous survey—your responses will be kept confidential and will be combined with others. This information is used only for research purposes.
It is important that you answer every item—the survey consists of approximately 40 items and should take about 20 minutes. Please respond to each item honestly and accurately. Thank you for both your time and consideration.
*A casual dining restaurant is a restaurant that serves moderately-priced food in a casual atmosphere. They typically provide table service, and may serve beer or wine or have a bar adjacent to the dining room. Applebee’s, Chili's, T.G.I. Friday's, and Olive Garden are typical examples of a casual dining restaurant.
