Introduction


You are invited to participate in a research study of green supply chain practices by people who lead supply chain organizations either in manufacturing or distribution. The study will provide supply chain leaders with information concerning the impact of green supply chain practices on operational performance.

You were selected as a possible participant because of your position in the organization. It is recommended that you have 5+ years of experience in supply chain management to complete this survey. Please read this form and ask any questions you may have before agreeing to participate in this study.

This study is being conducted by: Robert Price, Doctorial student at Warren National University.



Background Information:


The purpose of this survey is to determine the influences of green supply chain practices upon operational performance.



Procedures:


If you agree to be in this study, you are asked to complete 1. It will ask you to identify the the influences of generally accepted green supply chain practices. All questions are multiple choice and open ended questions with space for comments. It will take 5-8 minutes to complete depending on your choices and comments.

Risks:


Being a participant in this study has no foreseeable risks.



Benefits:


The researcher hopes to develop an understanding of the green phenomena as it relates to supply chain operations and then share this information with others who may benefit. This may help organizations such as yours develop more effective supply chain plans, and sustainability strategies.



Confidentiality:


The records of this study will be kept private. In any sort of report we might publish, we will not include any information that will make it possible to identify a subject. Research records will be kept in a secured file; only the primary researcher will have access to the records.
