You have been forwarded this form as part of the follow-up to the Entrepreneurship Training Workshop in which you participated 4 months ago.

The purpose of this form is to assess whether the entrepreneurship training has improved your business performance and changed the way you run your business, e.g. through improved entrepreneurial competencies. This information is very important for the follow-up of support needs and to increase the effectiveness of the training and business support services.

We would like to ask you to provide us with a few minutes of your precious time to fill-in the questionnaire. We would therefore like to assure you that all information provided will be kept strictly confidently and will not be shared with any third party or other business. The information will only be used for assessment purposes and in anonymous format (names and other possible identifications will be kept separately). We would therefore like to ask you to provide as much and accurate information for the questions as possible.
