Internet Service Provider Survey Questionaire

Thank you for your time in visiting the Internet Service Provider Survey Questionaire created by Tom O'Leary. You have been invited to participate in this survey so that I can find out more about what is important to you with regard to internet service providers. This survey is a necessary part of my study of marketing research and I sincerely value your help. Approximately 50 people will be asked to complete this survey and it will take approximately 5 minutes to complete and submit all of the questions.

Your participation in this study is completely voluntary. If you feel uncomfortable answering any questions, you can withdraw from the survey at any point. However it is very important for us to learn your opinions, so I hope that you will finish the survey if you can.

Your survey responses will be anonymous and confidential. The answers you submit will be coded and will not be linked to you in any way.
