You are invited to participate in our client survey. Spectrum Laboratory Network requests that you complete a survey that asks questions about our services and the quality of service we provide to you. It will take approximately five minutes to complete the questionnaire.


Your participation in this study is completely voluntary, but strongly suggested since only you can provide us with the feedback neccessary to help to improve our services to you. There are no foreseeable risks associated with this project. If for any reason you feel uncomfortable answering any questions, you can withdraw from the survey at any point. However, it is very important for us to learn your opinions.


Although you are asked to enter your office name as well as the city and state in which you are located, your survey responses will be kept completely confidential and data from this research will strictly be used to help us help you. Entering your office name simply allows our results center to separate problems and concerns to specific areas and regions. If you do not feel comfortable entering your office name, please provide us with the city and/or state. Your information will not be shared with anyone on an individaul basis, but simply be used in determining how your responses compare to other offices.
