To create a new Survey :-

1. Login
2. Click on My Surveys
3. Enter in a descriptive name for the new survey in the text field

After you register and name your survey, just select the question type you want. For example on the multiple choice question, tell the wizard how many answers you need and then enter the question and answer text... it will show you what you have entered. You can click "back" and change the text or specifications for the question. Click finish and the question will appear back in the builder...then add the next question type...

You can preview the actual questionnaire by clicking Preview.

What are the different question types available?

Virtually any question type you can imagine. To view samples of all the different question types that are available click on:

• Login »  My Surveys »  Edit Survey »  Add Question

Can I customize all the scales/text etc?

Yes -- All aspects of a question are customizable

Can I include Open-Ended Comments/ Other Option?

Yes. You can either add a separate question or include an "Other" option.
