This is a confidential assessment for older adults living in the Upper Peninsula, and the information you provide will be used to help us focus on services you need to help you remain in your own home or community for as long as possible. UPCAP will incorporate the input gathered from this survey into its draft of the 2017-2019 Multi-Year Plan for Services to Older Adults.
The Plan will be available for public viewing May 13, 2016, at www.upcap.org. Public hearings and Community Forums will be held March-June. Please check our website for dates and locations. All are welcome to attend.
