TESTING ACCOMMODATIONS REQUEST FORM

This form is for OAS-registered students who have been granted testing accommodations.

Student:
Please submit this form to notify AccessAbility Services of your need for testing accommodations for upcoming quizzes/exams. Please only submit this form after your professors have received your Letters of Accommodations. After this form is submitted, AccessAbility Services will contact your professor regarding your need for accommodations. Faculty members reserve the right to proctor their own exams if they are able to implement the accommodations. Please notify AccessAbility Services of any changes to exam dates.
