Purpose: The SDMS Foundation Hardship Grant Program provides grants to support sonographers who are temporarily disabled, unemployed, or affected by disasters, and are experiencing financial hardship which prevents them from renewing their SDMS membership and earning the CME credits required to maintain their sonography credential. Hardship grants will be used to directly cover the cost of one year of Standard SDMS membership and funds will be transferred directly from the SDMS Foundation to the SDMS.

Grant application periods: Applications for hardship grants will be accepted on a first-come basis while budget remains. 

Instructions: To apply for an SDMS Foundation Hardship Grant please fill in the information below as completely and accurately as possible. Be sure to upload the required attachments. It is your responsibility to ensure the application is completed fully - failure to follow instructions and properly complete the application will result in disqualification.

Notification: The SDMS Foundation will notify applicants regarding whether they have been awarded the grant within two weeks of application submission. All applicants will be notified via email.

Eligibility: To be eligible for an SDMS Foundation Hardship Grant, an individual must:
· Be a current SDMS Standard member in good standing within 90 days immediately prior to expiration or a recently expired (within 30 days); and
· Demonstrate need by fully completing the SDMS Foundation Hardship Grant - Online Application, including the required attachment(s). 
*NOTE: Only one SDMS Foundation grant (i.e. one Certification Examination Grant, Hardship Grant, Research Grant, OR SDMS Educational grant) and one SDMS Foundation Scholarship may be awarded to the same individual within a calendar year.
