It is the policy of Harrisburg University that all subjects participating in research read and sign an informed consent form prior to participation. Read the following carefully and acknowledge you understand the information by inserting your name below.

You have been informed that: 
 Dr. Tonya Miller, a professor in the Department of Physical Therapy at Harrisburg University, and her research team have identified you as a potential participant in a research study at this institution. 

 The purpose of this study is to describe the prevalence of competencies on the TTI TriMetrixDNA Assessment Tool demonstrated by practicing physical therapists. 

 Your participation in the study is completely voluntary and will involve completing the TTI TriMetrixDNA Assessment Tool. Participants will receive summary results of the assessment. 

 Participants do not receive direct benefit from participation, although there may be indirect benefit gained from a better understanding of their specific skill competencies as well as contribution to the body of knowledge related to skill competencies demonstrated by physical therapists. There are no apparent associated risks. 

 The results of this research may be published, but your name or identity will not be revealed. This research is confidential, which means that the research records will include some information about you, and this information will be stored in such a manner that some linkage between your identity and the responses in the research exists. You understand that the information will be kept confidential by limiting individuals’ access to the research data and keeping it in a secure location. No subject will be identified individually, and codes will be used to label data rather than names. The research team and the Institutional Review Board (a committee that reviews research studies to protect research participants) at Harrisburg University are the only parties allowed to see the data, except as may be required by law. If a report of this study is published or the results are presented at a professional conference, only group results will be stated. All study data will be kept for at least three years. Dr. Miller, TTI/Indigo Educational Company, and her research team will be the only people with direct access to the subject number decoding list. YOU UNDERSTAND THAT YOUR PARTICIPATION IS VOLUNTARY AND THAT YOU MAY WITHDRAW YOUR CONSENT AND DISCONTINUE PARTICIPATION AT ANY TIME WITHOUT PENALTY OR LOSS OF BENEFIT TO YOURSELF. In consenting, you are not waiving any legal claims, rights, or remedies. A copy of this consent form will be offered to you.
