Healthy IDEAS Innovation Readiness Assessment
 
 
Thank you for your interest in Care for Elders’ Healthy IDEAS (Identifying Depression, Empowering Activities for Seniors). Healthy IDEAS is an evidence-based program that integrates depression awareness and management into existing case management services provided to older adults.

Care for Elders and Baylor College of Medicine manage the dissemination of Healthy IDEAS to potential adopters. Before completing the survey it is important for you to have some basic knowledge of the Healthy IDEAS program. You may review written material or listen to recorded webinars by visiting the Care for Elders’ Healthy IDEAS website (http://careforelders.org/healthyideas). Who should take this survey: Case management providers serving clients in-home and by phone over a period of at least three to six months. Case management functions may be performed by persons with different titles including care coordinator, care manager, caregiver support specialist, and services coordinator to name a few. Case management may also be part of another service provided to individuals.
