Please consider your interactions with this individual (your line manager) over the past year while responding to each question.

Survey Instructions

- Some of you may have more than one line manager who you have directly worked with during the year of 2012. You need to finish all questions for one line manager before starting again with others.
- Using the rating scale, assign a rating to each statement based on your experience and direct interaction with the individual selected.
- Fill out all information to the best of your knowledge based on your observations and experiences.
- A rating (CR, 1, 2, 3, 4, 5) is required for all questions prior in order to submit a questionnaire.
- If you cannot rate an individual in relation to a specific statement, choose CR – Cannot Rate.
- Additional comments are not required, but allow you to provide additional developmental feedback.
