Madison County 310 Board Demographic Update:
The Department of Mental Health requires that we contact families and confirm demographics on an annual basis for those individuals currently on the waiting list for the Community Waiver Program in Madison County.
This update will be valid from October 2023 – October 2024.

Please complete the demographic fields below to update the information for the individual currently on the waiting list for services. Please note that this form can be accessed and updated at any time to notify us of a change in demographic or contact information for the individual.
Please note that if an individual is over the age of 18 and has not applied for SSI, please begin that process at Social Security. In the State of Alabama, an individual that qualifies for SSI, unless they have other disqualifying income or assets, will automatically be enrolled in Medicaid.
If the individual was placed on the waiting list prior to the age of 18 and is still in school, please ensure that you request the school to complete a Full Scale IQ test after the age of 18 and prior to the individual exiting school. Once an individual exits school there are very limited resources for obtaining the test without the individual incurring the cost.
