Patient Satisfaction Survey
Welcome to the Barrie and Community Family Health Team's (BCFHT) and the Barrie and Community Family Medicine Clinics' (BCFMC) Patient Satisfaction Survey. This survey is being conducted by Kholah Nisar, Ramneet Kaur, Sakina Nalwala and Sarega Thomas who are Research Analyst students at Georgian College. The purpose of this research is to gain insights that will help shape the development and improvements of BCFHT and BCFMC services and programs in order to improve patient satisfaction. The survey will take around 10-15 minutes to complete.
Your identity will be kept confidential and no identifying questions will be asked in the survey. However, some demographic questions will be asked (ex: ethnicity, age) and your family doctor’s name. The data collected will be stored in password protected laptops and files. The survey will not include any information that will make it possible to identify you in any reports or presentations made. The data will be presented in a combined format in presentations and reports (for ex: 58% of participants said xyz). The online survey platform used for this survey is QuestionPro, which stores their data in United States. The research will be used by the Barrie and Community Family Health Team (BCFHT) and Barrie and Community Family Medicine Clinics (BCFMC) to implement changes to services and programs to better serve patients.
Your participation in the research is completely voluntary. If any question makes you feel uncomfortable or causes discomfort, you may skip that question. You can exit the survey anytime you want by clicking the exit button on the top right-hand corner. However, whatever you have completed before exiting the survey will be submitted, and the researchers cannot delete that data as they will not know which data belongs to whom. Your decision to stop participating, or to refuse to answer any questions will not incur any penalty. There are no foreseeable risks or discomforts associated with this research.
