My name is Mikaela Felcher and I am a graduate student at Arkansas Tech University. I invite you to take part in a research questionnaire which will help with my study entitled New Madrid Seismic Zone Earthquake Response: How the National Guard Will Respond to a Catastrophic Quake. I am examining the connection between civilian and military disaster response agencies and emergency management organizations and seeking out any issues that may occur between the two. Taking part in this study is entirely voluntary. If you decide to participate you must sign this form to show that you want to take part. This research study is being conducted to explore perceptions of individuals in the civilian emergency management sector on the Arkansas National Guard’s role following a major earthquake in the NMSZ and identify the challenges of an integrated effort between military and civilian agencies. This research is important because it will help to determine if there are any conflicts with interagency coordination as well as policies and procedures. Identifying any issues will bring attention to areas of weakness that can hinder response and emergency aide to Arkansans in a time of crisis.
