| Hello! You are invited to participate in our survey to measure the quality of life in the Walnut community. We want to make sure projects being promoted by the Health and Wellness Initiative are relevant and important to our community. In this survey, approximately 4,000 people will be asked to answer questions about health and wellness. It will take less than ten minutes to complete the questionnaire. Your participation in this study is completely voluntary. There are no foreseeable risks associated with this project. However, if you feel uncomfortable answering any questions, you can withdraw from the survey at any point. It is very important for us to learn your opinions.
