Please complete the following form for a student who wishes to drop or withdraw from a Ferris State dual enrollment, concurrent enrollment, CARE 102, or Woodbridge Promise College Prep Program course(s).
ALL DROP/WITHDRAW REQUESTS MUST BE SUBMITTED BY THE HIGH SCHOOL/CAREER TECH CENTER INSTRUCTOR, GUIDANCE COUNSELOR OR ADMINISTRATOR. Drop/withdraw requests from students ARE NOT allowed.
Submit one form per class. If a student wishes to withdraw from two classes, this form should be submitted two times.
