You are invited to participate in our Advance Practice Provider Lifestyle Counseling Research Survey on Hypertension. In this survey, you will be asked to answer questions about lifestyle counseling as it pertains to your patients and hypertension. It will take approximately 5-10 minutes to completethe survey.

The American Heart Association (AHA) set a goal for cardiovascular health in the Unites States: “By 2020, to improve the cardiovascular health of all Americans by 20% while reducing deaths from cardiovascular diseases and stroke by 20%.” AHA asserts that in order to achieve its 2020 goal, healthcare providers would need to be comfortable with engaging in conversations with their patients to initiate the behavior modification process. 

It is very important for us to learn your opinions about lifestyle counseling. We are using this information to create a provider quick reference card that you can used at the point of care.  

Your participation in this study is completely voluntary. There is a small risk for breach in confidentiality when completing this survey.   However, no personal identifying data will be collected in the survey itself.  You can withdraw from the survey at any point.  Your survey responses will be strictly confidential and data from this research will be reported only in the aggregate.
