STEPS FOR MOVING YOUR OFFICE PHONE TO MICROSOFT TEAMS
This questionnaire is designed for faculty and staff.

This questionnaire will direct faculty and staff who are returning to campus through the proper steps to transfer their UTA phone number to Microsoft Teams and order a physical phone or headset.
 
The complete process to transfer individual office number includes three basic steps:
  1. Complete the "Convert your UTA office phone to Teams" form to move your phone number to our new telephone service provider. (This form is only used for individual numbers shared by one person)
  2. Set up an appointment through Microsoft Bookings a week before returning to campus to receive your new desktop phone or headset.
  3. You or a designee will need to be present when OIT delivers your new headset or desktop phone to your office during your scheduled appointment
 
Departments with shared extensions or lines answered by multiple people will need to submit a separate self-service ticket to the IT Help Desk.
