Campus Assessment Survey-COVID-19 Research

Dear Student,

I am an undergraduate researcher under the instruction of Dr. Dawn Ford, Master in Public Health and Chief Epidemiologist at the University of Tennessee at Chattanooga. We are conducting research regarding students’ opinions on receiving the COVID-19 vaccine and the pandemic in general.

I am requesting your participation, which will involve you to take a 5–10 minute survey. Your participation in this research is voluntary. If you choose not to participate or choose to withdraw from the study at any time, no penalty will be applied. If you choose to discontinue the survey at any time, your results will be discarded. The results of the research study may be published within our research; however, your name will not be used. The survey is anonymous and no identifying information will be collected.

There are minimal risks for this study. The potential benefits of the study include education regarding the public health of students at UTC and other educational institutions in Tennessee regarding questions in relation to the COVID vaccine.
