Hello: You are invited to participate in our survey [for Custodial Service]. In this survey, approximately [100] people will be asked to complete a survey that asks questions about Custodial Service. It will take approximately 5 minutes to complete the questionnaire.  
our participation in this study is completely voluntary. There are no foreseeable risks associated with this project. There are a few things I would like to make you aware of before taking this survey.  The first one is that the custodians job consists of cleaning, moving furniture, and set up of facilities as well as clean up.  We are not responsible for any painting, or maintenance such as heat or air.  Please keep that in mind when completeing the survey, Thank you.

How does the custodian react if you let them know something got missed when they cleaned your room?
 
They get an attitude with you.
 
They blame the other custodian.
 
They clean it up as soon as they can.
 
They take responsibility and take care of the problem as quickly as possible.
 
 
 
Every school room is on a schedule for deep cleaning.  Do you know what day your room is scheduled, or do the custodians leave you a note when it has been completed? 
 
I don't know when they come to deep clean and they don't leave a note.

The custodians are trying to adhear to the guidlines of Green Cleaning Processes that are mandated by the the State.  One of the mandates is to reduce waste so we do not remove toilet paper rolls or paper towel rolls until the roll is completely empty.  When a custodian is called to replace paper goods do they do it in a timely manner.
 
They don't seem to care and sometimes leave it for the next custodian.
 
Sometimes they get it, sometimes they don't, its hit or miss.
 
They normally take care of it as soon as they can.
 
We very rarely run out and they seem to stay on top of it.
